The term “entourage effect” was first coined by Dr. Raphael Mechoulam, a leading researcher and professor of Medicinal Chemistry at the Hebrew University of Jerusalem, in his 1999 study “Cannabis – The Israeli Perspective”. Dr. Mechoulam is widely recognized as one of the pioneering researchers in the study of cannabis and the endocannabinoid system.  He said the entourage effect refers to the idea that the combination of multiple compounds in a plant or combined from different plants can have a synergistic effect that is greater than the sum of their individual effects. In other words, when certain compounds are combined, they can work together to not only enhance each other’s therapeutic effects, but also modulate and reduce unwanted side effects.

One of the first plants in Western culture to have been identified to have an entourage effect is cannabis. While tetrahydrocannabinol is often considered the primary active compound in cannabis, research suggests that the presence of other compounds, such as cannabidiol and terpenes also modify and enhance the therapeutic efficacy of the plant.

The entourage effect has been observed in traditional Chinese herbal formulas for thousands of years, however it remains a topic of ongoing research and debate in the scientific community. The concept of the entourage effect is particularly relevant to TCM herbal formulas, which is where I learned about this synergistic effect of nutritive substances. In Oriental herbal traditions the herbal formulas often contain multiple herbs that are combined in specific ratios to create a balanced formula. The different herbs in a formula may contain many different active compounds, such as alkaloids, flavonoids, polyphenols, sterols, minerals, vitamins, or terpenes, that can interact with each other to produce a more powerful therapeutic effect by directing herbal actions toward certain organs and systems of the body.
